International Travel Requirements to enter the United States – Attestation Form and Collection of Contact Information

Regardless of the status shown on the VeriFLY app, mandated documents must be printed and filled out in its entirety for acceptance. 1. Passenger Attestation Form The Centers for Disease Control and Prevention (CDC) requires non-U.S. Citizens and non-U.S. Immigrant passengers (over two years of age) to submit a passenger attestation form to verify their vaccination status when entering the United States. Collection of Contact Information The Centers for Disease Control and Prevention (CDC) requires airlines to collect contact information from passengers before their arrival into the United States from a foreign country.
