Passenger Attestation Form The Centers for Disease Control and Prevention (CDC) requires non-U.S. Citizens and non-U.S. Immigrant passengers (over two years of age) to submit a passenger attestation form to verify their vaccination status when entering the United States. Collection of Contact Information The Centers for Disease Control and Prevention (CDC) requires airlines to collect contact information from passengers before their arrival into the United States from a foreign country.
